CBRS/CM/Peer Support
Community-Based Rehabilitation Services (CBRS)
CBRS helps individuals build the skills they need to manage daily life more independently. A CBRS specialist works with clients in their home or community to practice coping strategies, social skills, problem-solving, and routines that support recovery. The focus is on increasing stability, independence, and quality of life through hands-on, skill-based support.
Case Management (CM)
Case Management connects clients with the resources and services they need to support their mental health and overall well-being. A Case Manager helps identify needs, develop individualized service plans, and link clients to community supports such as housing, employment, medical care, and benefits. The goal is to reduce barriers, coordinate care, and empower clients to navigate systems successfully.
Peer Support
Peer Support is provided by individuals who have lived experience with mental health challenges and recovery. Peer Support Specialists offer understanding, encouragement, and hope by walking alongside clients as they work toward their goals. This service is rooted in shared experience and focuses on building trust, resilience, and a sense of belonging.
